Sibling Boarding Liability Waiver

I, the undersigned, acknowledge that I have requested my pets to be housed together in the same kennel during their stay at Michigan City Animal Hospital. While every effort will be made to ensure their safety, I understand that housing pets together may increase the risk of injury due to potential rough play, disagreements, or other unpredictable interactions between the animals.

By signing below, I agree that Michigan City Animal Hospital, its staff, and affiliates will not be held liable for any injuries or incidents that may occur as a result of my pets being housed together. I accept full responsibility for any potential risks and outcomes associated with this decision including costs of emergency services that may be incurred.

I also confirm that my pets are familiar with one another and that I believe they will be comfortable and safe in a shared environment.
